Andrea Mazziotti Working paper, October 2026

Marketing Specialist, ENT · Italy, Greece, Israel

ENT in Italy, Greece and Israel: my first 90 days

I applied for the Marketing Specialist role in the ENT team. A cover letter would tell you I can do the job, so I tried to do the first part of it instead: a market map, a plan for this year's business with the ApexCut launch, the long game for Stealth AXiS, a 90-day plan, a sample battlecard and an education programme for surgeons, all built from public sources in October 2026.

In brief

  • This year: I would expect most of the number to come from systems hospitals already own, NIM Vital and the IPC console with M5, MR8 and Skeeter, and from PROPEL, because every case uses single-use products. Usage per installed system is the first thing I would work on.
  • The launch: ApexCut blades, CE marked in April 2026, fit the StraightShot microdebriders already in the operating room. It is a conversion launch with no new capital, built with surgeons, scrub nurses and the tender team.
  • The long game: Stealth AXiS is entering Europe through spine surgery, so ENT should be in the platform business case from the first meeting, starting with the skull-base teams.
  • How to judge it: three 30-day phases, each ending with something the sales team can use.

Independent work prepared for a job application. Not produced, reviewed or endorsed by Medtronic. Product names belong to their owners and come from public sources. No clinical or economic claims are made on this page.

  • 24 April 2026 CE mark for ApexCut blades, made for the StraightShot microdebriders hospitals already use, after the US launch earlier in the year. [25] [26]
  • 8 June 2026 CE mark for Stealth AXiS in ENT: sinus and skull-base surgery, balloon procedures included. [1]
  • 10.9% Adults in Europe meeting the symptom criteria for chronic rhinosinusitis, from 6.9% to 27.1% across centres (57,128 responders). [8]
  • 19–22 May 2027 SIO national congress in Rome, with robotics and AI in ENT on the programme. [10]

02

Three markets that open through different doors

Italy buys through regions and lives with a device payback. Greece adds a hospital rebate and late payments. Israel buys through four health funds and registers devices on the back of American and European approvals. The same launch needs a different entry point in each country.

One clinical fact holds everywhere. Biologics are changing the treatment of nasal polyps, yet the 2023 EUFOREA/EPOS update keeps bilateral polyps after endoscopic sinus surgery as the main entry criterion for them [9]. Surgery stays on the pathway, and so does the technology that supports it.

Italy

How it buys
21 regional health systems. Device tenders are often run by regional purchasing bodies, with hospitals buying directly for smaller needs.
Pressure
The device payback (art. 9-ter, DL 78/2015) was upheld by the Constitutional Court in rulings 139 and 140, with company payments for 2015–2018 cut to 48%. TAR Lazio rulings in 2026 confirmed it. [12]
Compliance
Law 62/2022 creates Sanità Trasparente, a public register of transfers of value to healthcare professionals. At the last public update the platform was ready but not live. [13]
Signal
Italy hosted some of the first European Stealth AXiS cases in September 2026. [2]
Calendar
SIO 113th national congress, Rome, 19–22 May 2027. AOOI national meeting every autumn, the latest in Venice on 21–22 September 2026. [10] [11]

Where I would start: university hospitals with combined ENT and neurosurgery skull-base programmes, then the high-volume sinus centres.

Greece

How it buys
Public hospitals and EKAPY, the national central purchasing body for health, whose share is growing slowly. EOPYY reimburses devices used outside hospital.
Pressure
A rebate on devices bought by public hospitals (Law 5129/2024, art. 72). It does not apply to tenders that save more than 5% on the budgeted value, which changes how a bid should be priced. [14]
Payments
Overdue hospital liabilities reached €1.397 billion at the end of 2025, up from €1.164 billion a year earlier. [15]
Calendar
Panhellenic Congress of ORL-HNS, Athens. The 23rd edition ran from 26 February to 1 March 2026. [16]

Where I would start: accounts with budget visibility and a tender coming up. Here a tender-ready dossier is worth more than a brochure.

Israel

How it buys
Four health funds: Clalit, Maccabi, Meuhedet and Leumit. Clalit also runs hospitals and approves equipment through its own committee, while Maccabi owns the private Assuta network. [19]
Registration
AMAR registration relies on approvals from reference markets. Under the 2023 reform, when a device is registered in both the US and the EU, the FDA registration prevails. Stealth AXiS has been FDA-cleared for ENT since March 2026. [17] [18] [3]
Calendar
Annual meeting of the Israeli Society of Otolaryngology-HNS, traditionally in Eilat in early March, with about 300 ENT specialists. [20]

Where I would start: in a small and connected market a few academic centres shape practice for the whole country, so they get the first evaluations and the faculty roles.

03

This year is won on the systems already in the operating room

Medtronic does not publish ENT revenue by product, so this is my reading, to check in the first week. International ENT was growing at a high single-digit rate in the last quarter of fiscal 2026 [6]. Nerve monitoring and powered instruments put a system in the hospital and then use single-use products in every case, and PROPEL is single-use by design. Usage per installed system, and keeping those systems when tenders come round, is where I would expect most of the year to be decided.

Nerve monitoring

NIM Vital

The monitor stays in the operating room; the EMG endotracheal tubes, such as NIM TriVantage, and the electrodes are single-use [7]. In head and neck surgery NIM monitors laryngeal and facial nerve function, thyroid and parathyroid operations included, and in the same operations PTeye is used to detect the parathyroid glands [29].

My lever: usage per monitor, account by account, and one offer for the whole thyroid procedure, NIM and PTeye together.

Powered instruments

IPC with M5, MR8 and Skeeter

One console runs the StraightShot M5 microdebrider for sinus surgery, the MR8 high-speed drill and the Skeeter otologic drill. Blades and burs are the single-use part: every MR8 bur, for example, is single-use [7].

My lever: blades and burs per console, and the ApexCut conversion below.

Drug-eluting implants

PROPEL

Sinus implants that release mometasone furoate, in three shapes: PROPEL, PROPEL Mini and PROPEL Contour [7]. Unlike the other two lines there is no system to install: each implant is a sale.

My lever: from first use to repeat use, account by account, wherever the implant is available and reimbursed (question 3 in section 09).

The launch I would run first: ApexCut

ApexCut blades received the CE mark on 24 April 2026 [25], after a limited US release in February and a full US launch in May [26]. Medtronic developed them for less clogging and more efficiency, as blades for the StraightShot microdebriders hospitals already own [27]. So the launch needs no new capital: it is a conversion of the installed base, account by account.

  • Who: every account with M4 or M5 handpieces, ranked by sinus surgery volume and by the blades it buys today.
  • How: evaluations in real cases with the surgeon and with the scrub nurse, who prepares and changes the blades.
  • Contracts: a route into existing contracts in each country, agreed with the tender team: a technology update where the contract allows it, a new bid where it does not.
  • Claims: only Medtronic's validated wording, with bench tests and user surveys kept clearly apart.
  • Measure: the ApexCut share of the blades each account buys, month by month.

04

The long game: Stealth AXiS enters Europe through the spine, and ENT should follow it in

Stealth AXiS received the CE mark for spine and cranial procedures on 28 April 2026 and for ENT on 8 June [3] [1]. The first European cases, announced on 10 September, were spine cases in Germany, the UK, Italy and Spain [2]. On 15 October TIME named it one of the Best Inventions of 2026 [28].

In many hospitals neurosurgery or spine will evaluate the platform first, and the capital decision may be taken before anyone from ENT is in the room. I would join that conversation early instead of waiting for a separate ENT budget.

  1. Feb 2026FDA clearance, spine
  2. Mar 2026FDA clearance, cranial and ENT
  3. 28 Apr 2026CE mark, spine and cranial
  4. 8 Jun 2026CE mark, ENT
  5. 10 Sep 2026First European cases: 23+ spine procedures in 7 centres, Italy included
  6. NextENT cases in Italy, Greece and Israel

Move 1

Put ENT in every platform business case

Map each Stealth AXiS evaluation in Italy, Greece and Israel together with the Cranial & Spinal team, and bring the ENT use case to the first meeting. The system is modular, so a hospital can start with what it needs and add specialties over time [4].

Move 2

Start at the skull base

Endoscopic skull-base surgery is where ENT surgeons and neurosurgeons already operate side by side. Those teams connect the two budgets, and they are the natural faculty for the education programme.

Move 3

Offer the procedure, not the box

The AiBLE ENT ecosystem connects navigation with powered instruments, nerve monitoring and PROPEL [1] [7], and a procedure-level offer is easier to defend in a tender scored on quality. The direction is already visible in the US: ApexCut EM navigated blades, cleared in October 2026, connect powered instruments and navigation [26], and Stealth AXiS ENT is offered together with KARL STORZ endoscopy [5]. Whether and when both reach EMEA is one of my questions for headquarters.

In a real launch every clinical or economic statement would come from Medtronic's validated materials and go through medical and legal review.

05

Ninety days, three deliverables

Each phase ends with something the sales team can use, so the plan can be judged on what it produces.

  1. Days 1–30

    Listen before planning

    • Field days with the ENT reps in each country, in the operating room where hospitals allow it.
    • 25 structured conversations with surgeons, OR nurses, biomedical engineers and buyers, all on one interview guide.
    • An account baseline: procedure volumes by hospital (SDO discharge data in Italy, sales and tender data in Greece and Israel), installed systems by product family with single-use usage per system, competitor presence, tender calendar for the next 12 months.
    • Product, clinical and compliance training, MedTech Europe Code included.

Deliverable: account segmentation (A: skull-base and university centres, B: high-volume sinus centres, C: community hospitals), the ApexCut target list and a one-page diagnosis shared with sales and South Europe marketing.

  2. Days 31–60

Build the kit with the people who will use it

    • An ApexCut conversion kit for the reps: target accounts, an evaluation protocol for surgeon and scrub nurse, the contract route in each country and approved answers to the first objections.
    • A Stealth AXiS ENT value story for each stakeholder: surgeon, OR team, hospital management and procurement.
    • A tender toolkit, built with the tender and pricing teams: specification templates, a library of approved answers, scoring simulations for quality-weighted tenders in Italy and rebate-aware bids in Greece.
    • Battlecards for navigation, powered instruments and nerve monitoring, reviewed by headquarters, medical and legal.
    • The 12-month education calendar described in section 07.
    • Two or three reference centres selected for evaluations.

Deliverable: launch kit v1, ApexCut first, used by three reps in real accounts, with their feedback logged.

  3. Days 61–90

Prove it in the field, then make it repeatable

    • The first hands-on course in Italy and one in Greece or Israel.
    • A proposal for SIO 2027 in Rome: booth, symposium or hands-on format, with budget, built around the congress themes of robotics and AI.
    • An onboarding kit for new ENT colleagues: portfolio, procedures, competitors and compliance in ten short modules.
    • The first monthly report and the first input to demand planning.

Deliverable: a review with sales leadership of ApexCut conversion by account and of the Stealth AXiS ENT pipeline, and the 12-month plan.

06

A battlecard built only from public sources

A sample for one conversation: navigation in sinus and skull-base surgery. Real battlecards need validated claims and medical and legal review, so the last column lists the proof I would ask headquarters for.

Competitor What they lead with What a surgeon may say How I would answer, and the proof to request
Stryker Navigation on Scopis software with augmented reality on the endoscopic image and target-guided planning. The XprESS balloon can be tracked with a TGS guidewire. [22] “I can see the planned path on the endoscope.” One platform shared with spine and cranial, inside the AiBLE ecosystem. Proof: a workflow and planning comparison on sinus cases.
Brainlab Electromagnetic Kick EM navigation with no line-of-sight issues, an optical option, integration with the Olympus multidebrider. [23] “Our neurosurgeons already use Brainlab.” Bring neurosurgery into the evaluation and discuss the whole platform, robotics included. Proof: cost of ownership across departments.
Integra LifeSciences (Acclarent) TruDi navigation, balloon sinus dilation and the Aera Eustachian tube balloon. Acclarent was bought from J&J in a deal announced at $275 million. [24] “Navigation and balloons from one supplier.” Stealth AXiS ENT covers sinus balloon procedures [1], and the portfolio includes the NuVent Eustachian tube balloon [7]. Proof: the balloon workflow on Stealth AXiS.
KARL STORZ Endoscopy and visualisation. In the US, a Medtronic partner for integrated ENT solutions since June 2026. [5] “Will it work with my endoscopy tower?” Show the integration in the OR and treat them as a partner first. Proof: integration status and any EMEA plan.

07

An ENT academy a compliance officer would sign

In surgery, education is the fastest road to adoption and the most regulated part of this job. I would design it in three levels and check every format against the rules before the first invitation goes out.

Level 1

Fundamentals

For residents and young consultants: powered instruments, nerve monitoring and navigation. Case-based webinars in Italian, Greek and Hebrew or English, followed by a simulator or cadaver session.

Level 2

Advanced sinus and anterior skull base

For experienced surgeons. Hands-on courses in reference centres, with independent faculty and live case discussion.

Level 3

Faculty

For surgeons who will host visits and proctor colleagues. Train-the-trainer sessions and a shared case library.

Rules built in from day one

  • Third-party events go through the MedTech Europe Conference Vetting System before any support is offered.
  • Support to conferences goes through educational grants to institutions, never to individual doctors, as the MedTech Europe Code requires since 1 January 2018. [21]
  • In Italy, every transfer of value is designed to be publishable in the Sanità Trasparente register. [13]

08

What I would report every month

  • Single-use products per installed NIM and IPC system, by account
  • ApexCut share of the blades each account buys
  • PROPEL accounts, with first and repeat orders
  • Qualified Stealth AXiS ENT opportunities by country and value, evaluations started and converted
  • Surgeons trained by level, as reported by sales
  • Tenders entered and won on ENT lots
  • Use of sales tools: battlecards opened, tender answers reused, rep feedback score
  • Forecast accuracy on key product codes, as input to demand planning

09

Questions for my first week

Public sources stop here. These are the answers I would look for inside the company before committing to any number.

  1. Which M4 and M5 accounts already use ApexCut, and which blades does it replace first?
  2. Which accounts use NIM in every thyroid and parotid case, and which only in selected ones?
  3. Where is PROPEL available and reimbursed across the three countries?
  4. What is the installed base of IPC consoles, NIM monitors and StealthStation ENT, and when does each come up for replacement?
  5. Which Italian, Greek and Israeli centres are already in the Stealth AXiS plan for fiscal 2027, and for which specialties?
  6. How do the ENT and Cranial & Spinal teams share platform deals and their credit?
  7. When do navigated ApexCut blades reach EMEA, and does the US collaboration with KARL STORZ have an EMEA equivalent?
  8. Which tender criteria have cost us the most ENT lots in the last two years?

10

Why someone from outside medtech

I have never worked in medical devices. I have spent my career on the other half of this job: launching products and brands, building campaigns that have to sell, reading markets and competitors, and working next to the people who meet customers every day.

Three examples of that work. At EVO.it, an Italian provider of digital and connectivity services, I spent nine years next to the sales team: qualified leads grew by 40% while acquisition cost fell by 25%, and rebuilt sales materials cut the sales cycle by 15%. For Cosmoderma, a cosmetics contract manufacturer that sells B2B through five in-house reps, a new B2B website and targeted Google and LinkedIn campaigns turned occasional enquiries into a steady flow of qualified leads, enough to fill the plant. And before any of this I organised about a hundred events for university students, with thousands of attendees.

This page is the best evidence I can offer of how I would learn ENT: from primary sources, in the open, and with the sales team in mind.
